Short Term Plan - An alternative if you need immediate coverage for 30 days up to a year. A more economical alternative to COBRA when you are between health plans.
Enhanced Copay 80% PPO - A traditional PPO plan with benefits including doctor co-pays and Rx Drug co-pays. This is Humana's most benefit rich plan including unlimited in-network doctor visits paid by co-pay. A wide range of deductibles starting at $500 per member per year is available.
Copay 80% PPO - A lower cost traditional PPO plan with benefits including doctor co-pays and Rx Drug co-pays. This plan has a limit of six in-network doctor visits paid by co-pay per year per member and a lower $3 million lifetime maximum per person. Only two higher deductible levels are available.
Enhanced HSA 100% - A HSA-compliant plan that includes prescription drugs and an allowance for preventive tests. The Enhanced HSA 100% are "pure" High-Deductible plans with no cost sharing beyond the deductible when using an "in-network" provider. A wide range of deductibles are available.
Value 100% - A lower cost plan with a very high deductible ($5000 or $7500 individual) that is not HSA-compliant. This plan is only for folks that don't want to setup the tax-advantage Health Saving Account and are willing to have a much higher self-insured risk but a lower insurance cost. Rx drugs are covered after a $1000 additional deductible (deductible waived on generic drugs). Maximum lifetime limit per person is $2 million.
